Family HealthCare is proactively working to further develop the overall delivery of healthcare in our facility by becoming a patient-centered medical home.
The Patient-Centered Medical Home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into “what patients want it to be”. Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care. Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.
Family HealthCare is excited to have been recognized by the National Committee for Quality Assurance (NCQA) as a Level 3 (the highest and best available), Patient-Centered Medical Home.

What is Patient-Centered Medical Home (PCMH) Model?
The PCMH is a model of healthcare that utilizes fluid personal relationships between the patients themselves, their healthcare provider and personal care team. The patient has a single, medical “home” whether the medical needs are primary or secondary; preventive, acute or chronic care.
The PCMH model includes a personal healthcare provider and care team that will provide medical care. When specialty care is needed, as agreed upon in the patient/provider care plan, the FHC primary care provider and care team, and outside specialty provider, will enter into agreement to personally co-manage, facilitate and coordinate the care. This process is followed for all referrals to qualified professionals, such as hospitals, pharmacies and related community resources.
The PCMH model utilizes the patient-provider relationship to enhance primary care. In order to accomplish this, patients are encouraged to maintain engaged, active involvement in their own care. An agreed care plan coupled with patient satisfaction is the staple of PCMH. Patients who receive care at Family HealthCare will benefit from the following:
- Enhanced Access & Continuity: access to healthcare providers that are efficient in improved technologies that ensures continuity and communication with the medical community.
- Identify & Manage Patient Populations: providers and care team members identify, manage, and coordinate care for individual patients based on their conditions using evidence-based guidelines.
- Plan & Manage Care: comprehensive delivery of primary care through the improved coordination of care, managed by the patient’s personal healthcare provider and their care team.
- Provide Self-Care Support & Community Resources: provide patients with the educational resources they need to support themselves in effectively managing their own health.
- Track & Coordinate Care: resources to engage patients in the self-management of their health, coordinating their care across a number of specialty providers and organizations, and tracking tests.
- Measure & Improve Performance: a quality improvement structure in place that monitors the effectiveness of interventions, and identifies opportunities for improvement in clinical quality and patient experience.

What is Family HealthCare doing?
Family HealthCare works with healthcare providers, government entities, internal staff members and quality healthcare agencies to enhance and maintain the PCMH model within our facility.
Family HealthCare has PCMH specific goals and initiatives to study and maintain patient centered healthcare standards. Additional initiatives include the integration of quality improvement and care management processes along with patient educational tools and resources.
Family HealthCare works with external quality and support organizations to ensure efficiency and successful sustainability of PCMH standards. This enhances the primary care practice structure and improves clinical quality, outcomes, affordability and physician and patient satisfaction.
